To ensure a smooth visit, patients are reminded to:

• Arrive 15 minutes before the scheduled appointment time.

• Bring the completed new patient packet.

• Have insurance cards and a photo ID.

• Ensure a referral or authorization is provided if required by insurance.

• Bring all current medication bottles and relevant medical records (labs, EKGs, etc.).

• Bring the script from the primary care physician for the consult or tests.
