Patients are encouraged to arrive 15 minutes before their scheduled appointment time. Please bring the following:

• Completed new patient packet

• Insurance card(s)

• Photo ID

• Referral or authorization, if required

• Script from the primary care physician for consult or tests

• All current medication bottles

• Relevant medical records, including labs and EKGs

Insurance Referrals:

For patients whose insurance requires a referral or authorization, please ensure your primary care physician sends this document to Dr. Mendoza’s office prior to your visit. The office recommends checking the day before your appointment to confirm that the referral/authorization has been received.
